This free program helps residents transition from long-term care facilities back into a community setting. We provide individuals with a clear pathway regarding available choices for long-term service and support options for those who like to return home, or to another residential setting in the community.
Our staff works with the individual, the long-term care facility staff, family members, caregivers, guardians, community service providers, and others to ensure that transitioning individuals are connected to programs and services to help ensure the greatest level of independence possible in a community setting.

- Assist in providing linkages to community resources such as housing options, food stamps, low-cost cell phone
- assistance programs, utility assistance programs among other services available in your local community
- Assist in ensuring that you receive the public benefits you are entitled to receive
- Provide education on public assistance programs such as Medicaid and Medicare eligibility and application processes.
